We are seeking a dedicated and experienced Case Manager to join our TEAM!
NOTE: This is NOT a remote position
It is 100% in office.

The Case Manager is responsible for providing case management services to a variety of populations, following applicable State and Federal, and accreditation guidelines.
Responsibilities:The Case Manager is responsible for providing case management services to a variety of populations, following applicable State and Federal, and accreditation guidelines.
- Maintains contact with resources including schools, families, legal, employment, and referral sources
- Completes documentation and communicates with schools, discharge planning, FMLA/STD, courts, and return to work letters
- Communicates information provided by patients or other sources to the interdisciplinary team.

- Bachelors Degree in health-related field preferred.
- Minimum two years preferred experience in the medical and/or behavioral healthcare field
